As your primary care provider we will address your essential health needs from preventative medicine in the office setting to critical care in the hospital setting.

The visit will be conducted via electronic communication to interact with Dr. Sam with non-recorded video meetings hosted by PrognoCis Electronic Record System. All existing confidentiality protections under Federal and California law apply to information used or disclosed during this consultation.
